The Methylation Cycle: B Vitamins and Why MTHFR Matters
The supplement industry discovered “MTHFR” a few years ago and decided it was the secret behind everyone's health problems. Now I see ads claiming MTHFR mutations cause chronic fatigue, brain fog, infertility, and basically everything else, and of course there's a supplement for it. I decided to actually understand what methylation is, what MTHFR does, and whether the MTHFR marketing has any basis in reality.
Here's what the actual science shows about methylation, MTHFR, and B vitamins.
What Methylation Actually Is
Methylation is a biochemical process where a methyl group (one carbon atom bonded to three hydrogen atoms) gets transferred from one molecule to another. This happens thousands of times per second in your cells. Methylation is involved in neurotransmitter synthesis, DNA repair, myelin formation, cell membrane stability, and immune function.
The central hub of this system is the methylation cycle, which produces and uses S-adenosylmethionine (SAM), the body's primary methyl donor. The cycle requires: folate (B9), B12, B6, and betaine (from choline). Without adequate levels of these nutrients, the cycle slows and methylation reactions can't happen efficiently.
What MTHFR Is and What It Actually Does
MTHFR (methylenetetrahydrofolate reductase) is an enzyme that converts folate into 5-methyltetrahydrofolate (5-MTHF), the form that enters the methylation cycle. It's not some magic enzyme — it's one enzyme in one part of a much larger system.
About 30-40% of people carry a common MTHFR polymorphism (the C677T variant) that causes the enzyme to work at reduced efficiency. Some people carry two copies, meaning their MTHFR works at maybe 60-70% of typical efficiency.
The supplement industry decided this meant these people should all take 5-MTHF supplements and that MTHFR variants explain chronic illness. Here's the actual research: people with MTHFR variants do have slightly lower 5-MTHF levels on average. But most of them feel fine and have no health problems.
Does MTHFR Actually Cause Health Problems?
This is the critical question, and the honest answer is: MTHFR variants are associated with some conditions in some studies, but the evidence is weak and inconsistent.
MTHFR variants are associated with slightly increased risk of neural tube defects (but not as much as folate deficiency itself). Some studies link MTHFR variants to recurrent miscarriage, but other studies don't. Some link them to psychiatric conditions, migraines, or fibromyalgia, but these associations are inconsistent and the effect sizes are small.
The problem is that MTHFR is so common (30-40% of populations carry at least one variant) that if it were actually causing widespread disease, you'd see massive health disparities based on MTHFR status. You don't. Most people with MTHFR variants are healthy.
What the research actually shows: if you have an MTHFR variant AND folate deficiency, your methylation suffers more than if you had folate deficiency alone. But the solution is adequate folate intake, not 5-MTHF supplementation specifically.
Folate Deficiency vs. MTHFR Variants
Here's where the industry creates confusion. Folate deficiency genuinely impairs methylation and causes health problems. MTHFR variants might slightly increase your risk of problems if you're also folate-deficient. But fixing folate deficiency with regular folic acid works for most people, even those with MTHFR variants.
However, there's a subset of people with MTHFR variants who seem to respond better to 5-MTHF supplementation than to folic acid. This makes theoretical sense — if their MTHFR enzyme is inefficient, skipping that step by supplementing 5-MTHF directly might help. But this is an individual response thing, not a universal truth.
The research on MTHFR variants and psychiatric conditions, migraines, or chronic illness is much weaker than the supplement industry suggests. The evidence for treating MTHFR variants prophylactically (without demonstrated deficiency) is minimal.
B Vitamins and Methylation Support
What actually matters for methylation support: adequate folate (or 5-MTHF if you don't tolerate folic acid), adequate B12 (especially important for vegans and people with absorption issues), adequate B6, and adequate choline/betaine (from eggs, fish, and legumes).
Folate: The RDA is 400 mcg daily for adults. Most Western diets don't meet this from food alone. Folic acid is the synthetic form used in supplements and fortified foods; it works for most people. 5-MTHF is the active form; some people absorb or convert it better. Neither is inherently superior — it's individual.
B12: Required for methylation cycle function and neurological health. Deficiency causes fatigue, brain fog, and nerve damage. Vegans need supplementation (50+ mcg daily or B12-fortified foods). People with pernicious anemia need injections or high-dose sublingual supplements. Otherwise, food sources are adequate.
B6: Required for methylation and neurotransmitter synthesis. Deficiency is rare. Megadosing (>200 mg daily long-term) causes nerve damage. Adequate intake from food is fine for most people; supplementation helps if deficient or under high stress.
Choline/Betaine: Eggs are rich in choline. Beef, fish, and legumes too. Betaine is in beets. Most people eating a reasonably varied diet get enough. Vegans eating very low choline intakes might benefit from supplementation.
The Honest Assessment
If you have an MTHFR variant and feel perfectly healthy with normal energy, mood, and cognition, you don't need special supplementation. Your system is working fine.
If you have an MTHFR variant and also have fatigue, brain fog, mood issues, or migraines, ensuring you're adequate in B vitamins makes sense. But so does screening for other causes (sleep, stress, inflammation, nutrient deficiencies beyond just folate).
The idea that MTHFR variants are driving your illness and that expensive 5-MTHF supplements are the cure? That's marketing meeting anxiety, and it's not supported by the evidence.
The Bottom Line
MTHFR is one enzyme in one part of your methylation system. Having a variant doesn't mean you're broken — most people with MTHFR variants are healthy. Folate deficiency impairs methylation whether you have MTHFR variants or not. Adequate B vitamins (folate, B12, B6) and choline support methylation. Test for actual deficiencies before supplementing with expensive active forms. You probably just need to eat more leafy greens and eggs.
