Anti-Aging Supplements: What the Longevity Research Shows
Anti-aging supplements are a multi-billion-dollar industry, and for good reason — nobody wants to age. But when I dug into the longevity research behind the most popular anti-aging supplements, I found that some have legitimate science, many are built on preliminary animal research that doesn't translate to humans, and some are just pure hype dressed up with biohacking language.
Here's what the actual research shows about living longer and what supplements might help.
The Foundation: What Longevity Actually Requires
Before supplements, understand what the longevity research consistently shows matters: caloric restriction (or intermittent fasting), regular exercise (both cardio and resistance), sleep quality, stress management, social connection, and cognitive engagement. These are the variables that show up in every long-lived population studied.
Supplements are secondary to these. You can't biohack your way to 100 years old while sedentary, chronically stressed, and sleeping 5 hours. The supplements come after you've nailed these fundamentals.
NAD+ Boosters: NMN and NR (The Hype)
NAD+ (nicotinamide adenine dinucleotide) is a coenzyme involved in energy production and DNA repair. It declines with age. The theory: boosting NAD+ slows aging. NMN (nicotinamide mononucleotide) and NR (nicotinamide riboside) are supposed to increase NAD+ levels.
The research in animals is compelling. In humans? The picture is cloudier. Some human studies show NMN and NR increase blood NAD+ levels. Whether this translates to meaningful lifespan or healthspan extension in humans remains unknown. We don't have long-term longevity data yet.
The honest assessment: NAD+ boosters are a reasonable bet if you're interested in longevity optimization and can afford them. They're not dangerous at reasonable doses. But they're not proven to extend human lifespan. It's a “might help, won't hurt” scenario — if the cost doesn't bother you.
Resveratrol and Polyphenols (The Modest Effect)
Resveratrol (from red grapes, wine, berries) activates sirtuins — enzymes involved in longevity pathways. In animal models, this extends lifespan. In humans, resveratrol improves cardiovascular markers and might improve glucose metabolism. But human lifespan studies are lacking.
The broader category of polyphenols (quercetin, EGCG from tea, anthocyanins from berries) have antioxidant and anti-inflammatory effects. The research shows consistent modest benefits for cardiovascular health and cognitive function.
The realistic takeaway: eating foods rich in polyphenols (berries, tea, dark chocolate, grapes) is well-established to support longevity. Supplementing purified resveratrol or other polyphenols has research supporting cardiovascular benefits, but it's not clear they extend lifespan in humans. Food sources are probably better than supplements.
Metformin (The Interesting Case)
Metformin is a diabetes medication that's increasingly studied for longevity. In animals and cell models, it extends lifespan through multiple mechanisms (AMPK activation, reduced mTOR signaling). In humans with diabetes, metformin users live longer than non-users, but causation is unclear (maybe healthy people take it more consistently).
The TAME trial (Targeting Aging with Metformin) is ongoing, looking specifically at whether metformin extends lifespan in non-diabetic people. We don't have results yet. Using metformin solely for longevity without being diabetic is controversial — it's off-label and the evidence is incomplete.
My take: if you have prediabetes or metabolic dysfunction, metformin makes sense medically and might support longevity. If you're metabolically healthy, taking metformin for longevity alone is premature until the TAME trial concludes.
Senolytics: Fisetin and Quercetin (The Emerging Research)
Senolytics are drugs or compounds that clear senescent cells (old cells that accumulate with age and promote inflammation). Fisetin (from strawberries and cucumbers) and quercetin (from onions and apples) have senolytic properties in animal models. The theory: removing senescent cells slows aging.
The research is preliminary but intriguing. Human studies are minimal. There's a 2024 trial showing fisetin might improve physical function in older adults, but we're still in early stages.
Reasonable approach: getting fisetin and quercetin from food (strawberries, onions, apples) is definitely good. Supplementing concentrated forms is a “might help, low risk” scenario if you're interested in longevity optimization.
Rapamycin and mTOR Inhibition (The Prescription Drug Story)
Rapamycin is an immunosuppressant drug that's being studied for longevity because it inhibits mTOR (a nutrient-sensing pathway involved in aging). In animals, it extends lifespan. Some biohackers take micro-doses off-label. But rapamycin has serious side effects and is a prescription drug for a reason. Don't self-experiment with it.
Supplements That Consistently Show Longevity Support
Omega-3 fatty acids: Extensive research showing cardiovascular and cognitive benefits, reduced inflammation. DHA specifically is neuroprotective. 1-2 grams daily of omega-3 from food or supplement.
Vitamin D: Deficiency is associated with earlier death from all causes. Maintaining levels 40-60 ng/mL is associated with better long-term health outcomes.
CoQ10: Energy production support, cardiovascular health. Levels decline with age. 100-200 mg daily in older adults has research support.
Magnesium: Involved in 300+ enzyme reactions. Deficiency is associated with worse health outcomes. Adequate intake (or supplementation to reach ~400 mg daily) supports longevity markers.
What Doesn't Have Longevity Research
Most antioxidant megadoses: As discussed elsewhere, megadosing antioxidants doesn't extend lifespan and might impair adaptive responses to stress.
Collagen supplements: Popular for anti-aging, but the research on oral collagen absorption and actual effect on skin or joint aging is weak. Eating protein and vitamin C supports your body's collagen production better.
Hyaluronic acid supplements: Taken orally, HA is broken down. It doesn't reach your skin. The hype exceeds the evidence.
The Honest Longevity Strategy
The interventions with the strongest evidence for longevity: caloric restriction or intermittent fasting, regular exercise (150+ min of cardio + resistance weekly), 7-9 hours of sleep nightly, stress management, cognitive engagement, and social connection. These aren't sexy or pill-able, but they work.
Supplements that support these: vitamin D to adequate levels, omega-3 fatty acids, CoQ10 (especially if over 60), magnesium, and polyphenols from food. NAD+ boosters are a reasonable biohacking bet if budget permits. Avoid seductive theoretical compounds without human longevity data.
The Real Anti-Aging Approach
Lifespan is determined by sleep, movement, stress resilience, social connection, and cognitive engagement. Healthspan (quality years, not just quantity) is improved by the same plus consistent supplementation with vitamin D, omega-3s, and antioxidants from food. Flashy anti-aging supplements like NMN, resveratrol, and senolytics have preliminary research but unproven human longevity benefits. They're reasonable to try if budget allows, but they don't replace the unsexy fundamentals that actually determine how long you live.
